Evidentiary materials required for a notarization:

Notarization of birth certificate issued by a medical institute

A copy of the ID card, household register, and birth certificate of the applicant (a scanned copy is accepted)

A.    Notarization of household register

The materials generally required for a notarization of household register

1. The valid identity document of the applicant.

2. The household register of the applicant, or the original copy of his/her personal page (s) of Register Card for Permanent Resident and a photocopy of the first page in the institutional register attached with the employer’s official seal in the case of an applicant lives in an institutional household.

3. In case, the applicant needs to entrust other (s) to proceed with the notarization for him/her, the applicant shall provide the ID card of the entrusted, a hand-written power of attorney (indicating the reasons to entrust to whom it is entrusted for what type (s) of notarization, with the signature of the applicant, either the original copy or a faxed copy will be accepted).

B.    Notarization of education background, academic degree and transcript

1. The valid identity document of the notarization applicant

☆A natural person now living in China shall provide: (1) ID card; (2) Household register.

☆A natural person now living overseas shall provide: (1) Passport (a valid visa is required) or a Mainland Travel Permit for Taiwan Residents or Hong Kong and Macau resident ID card and pass or other foreign resident certificates; (2) Original household register or the evidence for the cancellation of a Chinese household registration.

2. Certificate or evidence of the education background and the academic degree

3. Transcript (issued by the school the applicant studies)

4. Photos: N+1 two-inch identification photos with the same original negative shall be provided for notarization of education background, here the letter N refers to the number of copies of notarization needed.

5. If not handled by the applicant himself/herself, a power of attorney and the ID card of the trustee shall also be provided.

C.    Notarization of nationality in China

To apply for a notarization of nationality, the applicant shall be a registered permanent resident of Beijing, or have the certificate such as Temporary Residential Permit or Work Residence Certificate of Beijing, or have a household register record or an archive record with the applicant's name (or former name used) in Beijing, in addition to the following evidentiary materials:

1. The valid identity document or passport of the applicant.

2. The household register of the applicant, or the original copy of his/her personal page (s) of Register Card for Permanent Resident and a photocopy of the first page in the institutional register attached with the employer’s official seal in the case of an applicant lives in an institutional household.

3.
